Hearing Loss in Older Adults: Signs You Shouldn’t Ignore

As a hearing care specialist, I meet older people every day who have lived for years with undiagnosed hearing loss. In many cases, it’s their children or other family members who first notice that “something isn’t quite right”, although they don’t always know how to act or what signs to watch for.

That’s why today I want to share the most common signs of possible hearing loss in older adults, how it affects their quality of life, and what we can do to help them.

Why is hearing loss so common in older adults?

Age-related hearing loss (presbycusis) is very common and progressive. It usually starts off mild, and because it doesn’t appear all at once, it often goes unnoticed. The problem is that, over time, this difficulty hearing affects communication, mood, and connection with the world around them.

Here are some of the most common signs family members tend to notice:

1. Turning the TV volume up high

This is one of the earliest signs. If they always say “the TV is too quiet” or need a volume that bothers everyone else, they may not be hearing certain frequencies well.

2. Asking you to repeat what you said

Another clear sign. If they constantly ask “what did you say?” or “say that again”, they may not be catching certain words clearly, especially in noisy environments.

3. Withdrawing from group conversations

Many older adults switch off during family gatherings because they can’t keep up with the conversation. This can lead to frustration, isolation and, at times, confusion with other conditions like dementia.

4. Not responding when spoken to from behind

If you speak to them from another room or without eye contact and they don’t respond, they may not be picking up the direction of the sound, or simply not hearing you at all.

5. Getting irritated easily

The discomfort of not understanding properly can lead to nervousness, misunderstandings or even anger. Sometimes it’s not that they don’t want to talk — it’s that they can’t follow the conversation.

6. Speaking louder than usual

Since they can’t hear themselves well, some older adults raise their voice without realising it. This change can also be a warning sign.

7. Avoiding noisy places Many older adults with hearing loss avoid bars, gatherings or places with background noise because they can’t follow what’s being said. They’d rather stay home than feel uncomfortable.

How untreated hearing loss takes a toll

When hearing loss isn’t caught in time, it can lead to significant consequences:

  • Social isolation: they stop taking part in conversations and activities.
  • Cognitive decline: hearing has been shown to be linked to memory and attention.
  • Depression and anxiety: the frustration of not being able to communicate can affect mood.
  • Falls and balance problems: the ear also plays a role in balance, especially in older people.

What to do if you suspect an older relative is struggling to hear

Here are some practical recommendations I often give in clinic:

  • Speak with empathy: Avoid phrases like “you never hear anything” or “I always tell you and you never listen”. Instead, try saying: “I’ve noticed you’ve been finding it harder to follow conversations lately — have you noticed that too?”
  • Don’t let it slide: It’s common to assume “it’s just normal with age” and do nothing about it. But the sooner you act, the better the results.
  • See a specialist: The first step is to visit an ENT doctor, who will carry out a medical assessment and a hearing test. With that report, we can then assess whether hearing aids would be beneficial. When they come to our centre, alongside another hearing test, we also carry out complete tests to assess speech understanding, among other things. This lets us see which hearing solution would work best given their hearing loss, their level of understanding and their daily activity.
  • Offer to go with them: Many older adults feel fear or resistance towards change. Going with them to the specialist and answering their questions can be key to helping them take that step. The most important thing is that they want to address it themselves, because if not, they won’t be willing to adapt — they need more time and to understand that this will improve their quality of life.

What if the diagnosis confirms hearing loss?

In that case, our hearing centre can arrange a personalised fitting. There are discreet, comfortable hearing aids suited to different levels of hearing loss. What matters most is that they feel comfortable and notice a real improvement in their daily life.

Some benefits of hearing aids for older adults

  • They start taking part in conversations again.
  • They regain confidence and independence.
  • Their mood and social relationships improve.
  • The risk of cognitive decline is reduced.

There are also limitations to bear in mind

Not everything happens immediately. Some people need an adjustment period to relearn how to identify certain sounds, and not every hearing aid suits every situation. That’s why a proper assessment and follow-up are so important.

Catching the signs of hearing loss in older adults early can make a real difference to their quality of life. If you notice any of the symptoms we’ve covered, my recommendation is to see an ENT doctor for a full assessment.

And if the diagnosis confirms it, at our hearing centre in Valencia we’ll help find the right fitting so your relative can get their wellbeing back.
