Ménière’s Disease | Causes, Symptoms and Treatments

Ménière’s disease is a chronic condition affecting the inner ear, disrupting both balance and hearing. In my experience as a hearing care specialist, I’ve seen how this condition, which usually appears in middle-aged people, can be very frustrating for those who have it, causing episodes of vertigo, fluctuating hearing loss, tinnitus (ringing in the ear), and a feeling of pressure or a “blocked ear”. Although there’s no definitive cure, there are effective ways to manage its symptoms and improve quality of life.

What causes Ménière’s disease?

Ménière’s disease is linked to excess fluid in the inner ear, known as endolymph. Although the exact cause of this build-up is unknown, it’s thought to be influenced by several factors:

  1. Inner ear drainage abnormalities: A blockage or malfunction in the channels that drain fluid from the ear can lead to fluid build-up.
  2. Viral infections: Some infections, such as labyrinthitis, can trigger the condition.
  3. Genetic factors: There’s a genetic predisposition in some families.
  4. Autoimmune reactions: The immune system may attack the cells of the inner ear, triggering the condition.

Knowing these possible causes helps us identify more personalised treatment options.

Symptoms you shouldn’t ignore

Symptoms of Ménière’s disease can vary in intensity and duration. The most common include:

  • Vertigo: A spinning or moving sensation that can last from minutes to several hours. Nausea or vomiting are common during an episode.
  • Hearing loss: Usually fluctuating and mainly affecting low frequencies. Over time, it can become permanent.
  • Tinnitus: Ringing or noises in the ear, ranging from a soft hiss to a more intense sound.
  • A feeling of pressure or fullness in the ear: Many people feel their ear is “blocked” or under pressure.

If these symptoms are getting in the way of your daily life, don’t ignore them. Catching them early is key.

Treatment options: what can I do?

Treatment for Ménière’s disease focuses on relieving symptoms and improving the patient’s quality of life. The most common options include:

  • Hearing aids: A solution to improve hearing and manage tinnitus.
  • Medication for vertigo: Such as betahistine or diuretics, which can help reduce the amount of fluid in the ear.
  • Dietary changes: Cutting back on salt and caffeine can reduce fluid retention and, therefore, symptoms.
  • Vestibular rehabilitation therapy: Helps improve balance and reduce the feeling of dizziness.
  • Intratympanic injections: Corticosteroids or antibiotics can be injected into the middle ear to relieve symptoms.
  • Surgery: In severe cases, surgical procedures can be carried out to relieve fluid pressure in the inner ear.

How do hearing aids help?

Hearing loss caused by Ménière’s disease can significantly affect your quality of life, making communication in social settings difficult. Hearing aids are an effective solution for improving hearing and countering these challenges.

That said, fitting hearing aids can be tricky because of the fluctuating hearing typical of the condition. Because of this, regular adjustments are needed to keep them working at their best. What’s more, noise sensitivity in the affected ear can be intense, so it’s a good idea to use suitable hearing aids and add extra hearing protection with personalised devices in noisy environments.
